Japan is lagging behind in vaccine rollout just weeks before the scheduled opening of the 2020 Tokyo Olympics, with only about 1.9 percent of the population fully vaccinated so far.

The U.S. State Department on Monday urged against travel to Japan, saying the country will now fall under a "Level 4-Do Not Travel" advisory. Several areas in Japan are under a state of emergency and the nation's labored vaccination campaign is not helping.

With infections rising and nearly half the country's 47 prefectures under some kind of emergency measures, vaccinating the population has become a top priority.

"We have to book vaccines by calling, and the line gets crowded and makes it difficult to book. They should have thought about this more," one of the residents said.

"With the Olympics, I think we need to get vaccinated fast and prevent the spread of infection," another resident said.

Japan is behind many other developed nations with its vaccination rollout. Inoculations for the elderly only began in late April and there is no target yet for the general population.

The country requires domestic clinical trials for overseas vaccines, which has slowed down the approval process. Pfizer was approved in February, and Moderna and AstraZeneca received approval in mid-May.

Japan has also been hampered by distribution issues. William Ishiawata has been leading the COVID-19 response for a clinic in Miyazaki prefecture. He says the process has been slowed by confusion and a lack of expertise among local government bodies tasked with the rollout.

"Specifically in our city, they have asked the local medical association to staff the vaccination center, which does make sense, but it's not their job. In no way is it their job to find staff for vaccine distribution. That's a government job, not a local body job," said William Ishiwata, Clinic Administrator of Nadeshiko Internal Medicine.

A chronic shortage of medical workers, even before the pandemic, has exacerbated the problem. Nurses in the worst-affected areas are working overtime and many are becoming increasingly exhausted.

Covid-19 cases remain high and health experts say the state of emergency could yet again be extended. Japan still needs to vaccinate more people to ensure a successful Olympic Games.

The 2020 Tokyo Olympics are scheduled to take place from July 23 to August 8 and the Paralympic Games from August 24 to September 5.
